KHEDF SCHOLARSHIP ACCEPTANCE FORM - New Scholars ONLY

This form serves as your official acknowledgment and acceptance of the awarded scholarship. By completing this form, you agree to meeting the requirements outlined in the scholarship terms, including active participation in designated events and programs. Your dedication to these commitments will not only maximize your personal growth but also contribute to the positive impact of the scholarship program. Please carefully review and complete the form to signify your acceptance and commitment to the terms and conditions. Congratulations once again on your well-deserved scholarship!

Failure to submit form by deadline may result in forfeit of scholarship.

ENROLLMENT VERIFICATION FORM (EVF)

September 1st is the deadline to submit the EVF for the fall semester and February 1st is the deadline to submit the EVF for the spring semester. (BE AWARE OF YOUR COLLEGE'S SPECIFIC TUITION DEADLINES)

The Enrollment Verification Form (EVF) serves as a document for KHEDF to ensure that our students meet their scholarship requirements.  At the beginning of each semester (Fall/Spring), scholars must submit the form to verify grade point average(GPA), Major, and most up-to-date contact information.

1. Complete the Enrollment Verification Form (EVF) after you are officially enrolled, tuition for the semester you are receiving your scholarship has been billed on your student account and grades have been posted. (For Fall Semester EVF's this refers to grades for Summer Classes, if no summer classes were taken, please ignore this specific step. For Spring it refers to after grades have been posted for the Fall Semester.)
2. Make sure your unofficial transcript screenshots show your Full Name, GPA, and current courses enrolled in.
3. After the form is received by KHEDF, a check for the semester funds will be mailed to the student’s account. 
   
4. Please allow up to One(1) week for KHEDF to process and Two(2) weeks for Financial Aid to post funds on your account.

SCHOLARSHIP PROBATION PLAN

KHEDF Scholars who do not meet the required GPA, must complete the Scholarship Probation Plan to be eligible to not forfeit scholarship. 

KHEDF GRADUATING SCHOLARS

KHEDF Scholars who will be graduating, join us in celebrating your success! We would like to highlight your accomplishments by providing you a KHEDF graduation cord to wear across the stage. Submit this form to ensure you receive your cord before your graduation date.

Scholarship funds will be posted on your student account depending on the scholarship you received.
